• Phone and Alternate Numbers – Primary and backup contact numbers.
• Caller Type – Contractor, Homeowner, Municipality, Government, Utility, or Other.
• Company – Organization, association, or homeowner’s name.
• Caller– The individual making the request. New callers trigger the “New Caller Survey.”
• Address / City / State / ZIP – Official address of the company or homeowner. ZIP auto-fills city/state.
• Email – Strongly encouraged; provides confirmation and Positive Response notices. Must always be verified by spelling back to caller.

Context: This information forms the legal record. Under updated law, excavators must give a valid notice of intent to dig between 3 and 12 working days before starting. Without complete caller information, NC811 cannot enforce compliance.
