Location Information
- County and Place – Required for jurisdiction. If unincorporated, place defaults to county.
- Subdivision / Lot Numbers – Capture precise development data. No more than 5 adjoining lots or ¼ mile per ticket.
- Street and Cross Streets – Main location of work; up to two cross streets for accuracy. If no street name exists, enter “UNKNOWN” and describe directions in Locate Info.
- Sub-division - planned development where a large tract of land is divided into individual lots for residential construction,
- Lot – A plot of land
- Locate/Stake Info Box – Clear description of work area (e.g., “Locate front yard between house and street, both sides of driveway”).
- Blasting / Boring / Railroad / White-Lining – Required yes/no questions.
- Size Requirement – Confirms whether area is ≤ 5 parcels or ¼ mile.
- Duration- how long the excavation will occur
- Work Type and Done For – Specific excavation activity and beneficiary of the work.
Context: Law requires excavators to clearly describe or white-line the work area. White-lining is mandatory if the caller cannot provide a clear description.
